Parent / Guardian Consent
A parent/guardian consent must be completed and submitted for any Polk County student under the age of 18 to have their content featured on the Be The Future Be U™ website. Submissions will not be published without parent/guardian consent. Please include parent email in the message portion of the contact form to be contacted for consent.
